We’re so glad you’re here. Beginning services for your child is an important step, and our team is here to make the process as smooth, supportive, and stress-free as possible.
Below you’ll find everything you need to get started. Our intake packet helps us learn about your child, understand their strengths and needs, and ensure we can create the most effective ABA program for them.

Once you’ve completed the forms, simply email the full packet and required documents to RSCIntake@readysetconnect.org.

What You’ll Need to Complete & Submit
Intake Packet:

Intake Form (contact details, insurance info, signatures)

Privacy Notice acknowledgment

Emergency/Health Form
Release of Information Form:

Completed and signed (allows us to coordinate care and request/send records as needed)
Required Attachments:

Diagnostic evaluation

Letter of medical necessity from your child’s physician

Copies of insurance cards (front & back)

Verification of ABA coverage from your insurance provider

Any applicable school or service documentation
It’s very important that all forms and required documents are filled out completely and submitted before your appointment date. This includes personal information, insurance details, and all necessary signatures. Having everything ready in advance helps us begin services for your child without delays.
How to Complete a Fillable PDF
If you're using a computer:

Download the PDF to your device.

Open it using Adobe Acrobat Reader (free download) or your computer’s built-in PDF viewer.

Click into each box to type your responses.

For checkboxes, simply click the box to mark it.

Add your digital signature if prompted, or print/sign/scan if preferred.

Click Save after completing each page.
